Paying for Benefits While on Personal Leave If you are on a Personal leave, your medical, dental, vision, and Flexible Spending Account coverage will end on the last day of the month in which you start the leave. Once you return from the Personal leave, your benefits will automatically restart on the first of the month following your return to work, if your Personal leave does not exceed twelve months and if you are still considered benefit-eligible due to the most recent Hours Audit. If you would like to continue your benefits while on a Personal leave, contact the Hyatt Benefits Center (877-629-1234) to enroll in COBRA coverage for the duration of your Personal leave. If you enroll in COBRA coverage, you will be responsible for making the full premium payment for the coverage(s) you elect, plus a 2% administrative fee, to the Benefits Center. The Benefits Center will advise you of the COBRA rates, the payment process, and deadlines when you elect COBRA. If you are enrolled in the High Deductible Health Plan (HDHP) with Health Savings Account (HSA), your HSA account remains available to you during the leave. While you won’t be making any pre-tax contributions via payroll deductions during your Personal leave, you can opt to make post-tax contributions directly with Optum, if desired. Additionally, for any month in which you are on a leave status as of the first of the month, you will not receive the pro- rated annual employer contribution for that month. Your life and disability coverages will continue for up to a maximum of eight weeks. You will be responsible for making payment arrangements with your HR department for your monthly premiums for Supplemental Group Term Life, Dependent Life, Short-Term Disability Buy Up, Voluntary Long-Term Disability, and/or Permanent Whole Life Insurance. In the unlikely event that your Personal leave exceeds eight weeks, your life and disability insurance coverages will end. If your Personal leave was preceded by the maximum periods of medical leave, your life and disability insurance coverage will not continue for the eight weeks. If your life and disability coverages were ended due to exceeding the maximum of eight weeks on Personal leave, your Basic Group Term Life Insurance, Hyatt-Paid Short-Term Disability, and Hyatt-Paid Long Term Disability coverages will be reinstated upon your return to work, if your Personal leave time does not exceed twelve months and you are still considered benefit-eligible because of the most recent Hours Audit. You must re-apply for Supplemental Group Term Life, Dependent Life, Short-Term Disability Buy Up, and Voluntary Long-Term Disability insurance, if you wish to re-enroll in these coverages.
